Telephone Request (Move, add, drop, change)

To report an issue with an existing telephone hardware or settings, submit a Something Broken form.

What Is It?

  • Add Hunt Groups or Call Pick-Up Groups
  • Add Additional Extensions to a Phone
  • Remove Extensions from a Phone
  • Change Caller ID Name
  • Add, modify, or Remove Voicemail

Steps to Request

  1. Title: Provide a brief title that summarizes the issue or request.
  2. Email of Requestor: Enter your email address so we can communicate with you.
  3. Contact Number: Include a phone number where we can reach you.
  4. Building: Specify the building or location related to the request.
  5. Room Number: Indicate the room or area associated with the request.
  6. Current Extension (if applicable): If you have an existing extension, provide it here.
  7. Impact Level: Specify whether the issue affects an individual, group, department, or organization.
  8. Full Details/Justification: Describe the issue or request in detail. Be specific and concise.
