2. Requestor’s Email: Enter the email address of the person making the request.
3. Contact #: Specify a phone number for direct communication.
4. Can we contact you on Teams?: Indicate whether communication via Microsoft Teams is acceptable.
5. Hours of Availability: Specify the preferred time slots for communication.
6. Location: Identify the relevant physical or virtual location.
7. Category: Classify the request into relevant categories (e.g., technical support, access request, etc.).
8. Full Details: Include all relevant information related to the request.
