Purpose:
This form is intended only for faculty and staff requesting departmental use of software that is already approved, currently in use by another department, and listed in the university’s software directory. Approval of this use case form authorizes the requesting department to use the software; however, it DOES NOT provide software licenses or technical support. Securing the appropriate licenses remains the sole responsibility of the requester.
Pre-requirements prior to form submission:
- Visit the Software Directory
- Confirm the software you need is listed in the software directory and your department is not showing as an approved department

- Click on Add your department button

- When submitting your USE CASE Form, please make sure every question is answered and include as much detail as possible. The more information you provide, the better we can understand and assess the use case.
- The Information Security Office will receive your request and review it accordingly.
**NOTE:
- If the software states "ALL", please do not submit this form.

Reasons for Automatic Use Case Decline:
- Wrong software/technology is selected
- Submission was created by a student
- Your department is already listed as an approved user
