What Is It?

The Telephone Management Services Request Form is designed to assist users with various telephone settings and configuration changes. This includes setting up call forwarding, accessing voicemail, updating phone settings, adding Hunt Groups or Call Pick-Up Groups, adding additional extensions to a phone, removing extensions from a phone, changing Caller ID name, and adding, modifying, or removing voicemail.

Steps to Request

  1. Select the blue Request Service button on this page.
  2. Email of Requestor: Enter your email address so we can communicate with you.
  3. Contact Number: Include a phone number where we can reach you.
  4. Current Extension (if applicable): Provide your assigned office phone number.
  5. Building: Specify the building or location related to the request.
  6. Room Number: Indicate the room or area associated with the request.
  7. Impact Level: Specify whether the issue affects an individual, group, department, or organization.
  8. Services Needed: Select the services you are requesting.
  9. Description: Describe the request in detail. Be specific and concise.
