Steps to Request
- Select the blue Request Service button on this page.
- Title: Provide a brief title that summarizes the issue or request.
- Email of Requestor: Enter your email address so we can communicate with you.
- Contact Number: Include a phone number where we can reach you.
- Building: If you’re off-campus, specify the building you would report to if you were on campus.
- Room Number: Indicate the room or location related to the request.
- Impact Level: Specify whether the issue affects an individual, group, department, or organization.
- Computer/Device Asset Tag Number (if applicable): If the request involves a specific device, provide its asset tag number.
- Full Details: Describe the issue or request in detail. Be specific and concise.
